Nursery Parental Declaration Form for Early Years Entitlement
Dear New Nursery Parent/Carer
Your child will be coming home on Wednesday 16th September 2026 with a parental declaration form to confirm their early years entitlement. The form is a declaration of what entitlement you will be taking up at King Edward Primary School.
Can we please ask ALL parents to complete the form and hand it back into the school office along with your child’s birth certificate or passport so we can verify your child’s identity and their date of birth. All completed forms must be handed into the school office by Wednesday 23rd September 2026. If you are unable to call into the school office, you can hand the completed form to the Nursery/OOSC staff and you can email us a copy of the birth certificate or passport to info@kingedward.org
